Supplemental cancer, heart, accident, and ICU coverage in Dallas–Fort Worth

Four optional supplemental policies

Each policy is separate and optional. You may select one, combine several, or omit a policy, subject to availability and eligibility. Intensive Care (ICU) is its own policy and is not automatically included in the others.

What these policies are, and are notThese are supplemental, limited-benefit policies. They are not major medical insurance and are not a replacement for health insurance or workers' compensation. Benefits are paid only for qualifying events under the policy terms, and proof of loss is required.
  • Cancer

    Cash benefits for covered cancer-related events and treatments as defined in the policy, subject to its limits, exclusions, and eligibility rules.

  • Heart Disease, Heart Attack and Stroke

    Cash benefits for covered heart disease, heart attack, and stroke events as defined in the policy. A qualifying event and proof of loss are required.

  • Accident

    Cash benefits for covered accidental injuries as defined in the policy. Exclusions apply. In the Texas sample accident form (A8), exclusions include some aviation-related activity, such as serving as aircraft crew.

  • Intensive Care (ICU)

    Cash benefits when a covered hospital stay includes intensive care, as defined in the policy. This is a separate, optional policy.

Benefit levels: Elite8 and Preferred4

The two options differ in benefit levels, but four-unit benefits are not necessarily exactly half of the eight-unit amounts. Exact benefits and premiums require the applicable schedule and a quote. Availability and eligibility apply.

Examples of limits in eight-unit Texas sample forms

These are illustrative eight-unit Texas sample figures. They are not current offers, quotes, or a guarantee that any plan is available, and each applies only under the form named and its benefit conditions.

  • Cancer, form C15: self-administered chemotherapy benefit of $400 per qualifying calendar month, with a $48,000 lifetime maximum, subject to the form's benefit conditions.
  • Cancer, form C15: radiation planning has a $2,400 lifetime limit, and special treatment has a $4,000 lifetime limit.
  • Heart, form H7: the heart surgery and recovery benefit has a $24,000 per-hospitalization limit.
  • Accident, form A8: hospital benefits are limited to 180 days per accident.
  • ICU, form I10: intensive care benefits are limited to 30 days per hospitalization.

Some provisions do not state a lifetime aggregate, but that does not make any benefit unlimited. Every benefit is subject to its own terms, conditions, and exclusions.

Cash value and maturity benefits

Some policies include Cash Value Surrender and Maturity benefits as part of the policy itself. Benefits are calculated separately under each policy. The maturity benefit equals premiums paid while the policy was in force, minus Claims Incurred before maturity, as defined in that policy.

  • Maturity, issue age 40 or younger: the later of the 25th policy anniversary or receipt of 25 full years of premium.
  • Maturity, issue age 41 or older (sample cash-value forms): the policy anniversary after reaching age 65.
  • Early surrender: available only after six completed years of premium payments. The amount equals premiums paid while the policy was in force multiplied by the applicable cash-value table percentage, minus Claims Incurred, as defined in that policy. It is not a full refund beginning in year six.
  • At maturity: you do not have to surrender the policy to receive the maturity benefit, and coverage can continue under policy terms.

Rider premiums are included in early surrender only if the rider was in force for the entire period the policy was in force, and at maturity only if the rider is in force on the maturity date, subject to the applicable policy terms.

Cash-value options are for enrollment at age 55 or younger. Older applicants may have different policy forms with different benefits, so ask me about age-appropriate options.
